1. Scope of these terms
These terms set out the scope and type of services provided by Pracownia Terapii i Rozwoju HEFIO Paulina Gorazdowska and the conditions under which they are provided.
2. Scope of services
The practice provides the following services:
- psychological consultations,
- individual therapy,
- dyadic therapy,
- counselling for parents,
- sexological counselling,
- group workshops and psychoeducation.
3. Where services are provided
Services are provided in person at the practice at ul. Śląska 66b in Gdańsk and online.
4. Requesting a service
Services are provided at the request of the Client (an adult) or of a parent/legal guardian (in the case of minors).
5. Acceptance of these terms
Booking an appointment constitutes acceptance of these terms.
6. Conditions for using the services
The services may be used by persons with full legal capacity. For minors, the consent of a parent or legal guardian is required. For persons over 16 years of age, “double consent” (of the child and the parent) applies.
7. Information provided by the Client
The Client or legal guardian is obliged to provide truthful information necessary for appropriate help to be given.
8. Personal data
The controller of personal data is the entity running the practice. Data is not shared with third parties. Records kept during sessions are available to the therapist only.
9. Professional confidentiality
The psychologist is bound by professional confidentiality in accordance with applicable law. Confidentiality is the foundation of a proper relationship with the Client and of practising a profession of public trust.
10. Confidentiality for children and young people
The psychologist also guarantees the right to confidentiality to children and young people. Information obtained from a child may be disclosed to a guardian only if the child’s health, safety or life is at risk.
11. Booking appointments
Sessions take place at the agreed time – please arrive just before your appointment. If you are late, the session is not extended and the fee remains the same.
12. Cancellations and payments
- The services are paid.
- An appointment can be cancelled no later than 12:00 noon on the day before the session.
- Cancellations should be made by text message to the practice’s contact number.
- If an appointment is not cancelled in time, the full fee for the consultation is payable.
- Payment can be made:
- by bank transfer (no later than the day before the session),
- in cash at the practice on the day of the session.
- If payment is not made on time, the practice may refuse to provide the service.
- The psychologist is likewise obliged to give notice of any cancellation of a session and to arrange a new date.
13. Prices
- Psychological consultation / Individual therapy – 250 PLN
- Dyadic therapy (couple, married couple, parent–child, siblings) – 250 PLN
- Issuing a document – from 250 PLN
- Workshops – price agreed individually
Session length: 50 minutes.
14. Responsibility for minors
Parents or guardians are responsible for the safety of children and young people outside the therapy room (in the waiting room, in the building, and on the way to and from the practice).
15. Final provisions
These terms apply from 1 January 2025 until revoked. The practice reserves the right to change these terms and the price list with one month’s notice.
